Jeffrey R. LeSueur, M.D.
Dr. Jeffrey R. LeSueur was born in Arizona and spent his early childhood on various Army bases while his family was on active duty. Eventually, they settled in Sierra Vista, Arizona, where he attended high school. After serving a two year mission for his church, he went on to graduate Magna Cum Laude with Honors from the University of Arizona. He received his medical degree from the prestigious Baylor College of Medicine in Houston, Texas, and completed his surgical residency training in Otolaryngology at the University of Tennessee Health Science Center in Memphis, Tennessee. Dr. LeSueur and his wife, Erika, relocated to the White Mountains with their four children in 2012. In his free time, Dr. LeSueur enjoys outdoor activities, biking and water sports.
Dr. LeSueur enjoys treating patients of all ages with medical problems or concerns related to the ear, nose and throat. Each patient brings unique symptoms and challenges, and he is dedicated to providing a caring and personalized experience for all patients. Dr. LeSueur strives to make certain every patient leaves his office meeting his or her expectations.

Dr. LeSueur is Board-Certified in Otolaryngology-Head and Neck Surgery and enjoys treating children and adults with problems and concerns relating to the ear, nose and throat. The specialized skills that Dr. LeSueur brings to the community provide greater access to ENT care in the White Mountains.

General Information
Appointments
We see patients by appointment only and do not accept walk-ins. Referrals from a primary care physician are encouraged, as documentation of chronic conditions is needed for many surgeries to be approved by insurance companies. Urgent situations require a referral, as the doctor needs to review all available information in order to determine appropriate emergent scheduling. All new medical problems must be evaluated in the office and cannot be diagnosed or treated over the phone. As a new patient, please arrive 30 minutes early. New patient forms can be downloaded, printed, and filled prior your appointment. Please be prepared on the day of your appointment by bringing your insurance card, picture ID, co-payment and any pertinent medical records.

Cell Phone Policy
While in our office, we ask that you turn off your cell phone in consideration of other patients and to avoid disruption.
Notice of Significant Beneficial Interest
It may be decided, during your appointment with Dr. LeSueur, that you require surgery for treatment and will be scheduled in the near future. Dr. LeSueur does surgery at Summit Healthcare Regional Medical Center and Sunrise Ambulatory Surgical Center. If your surgery is scheduled at Sunrise Ambulatory Surgical Center please be aware of the following:
Arizona Business and Professions Code, and the Centers for Medicare and Medicaid Services (CMS) require that you be notified when your physician, or someone in his or her immediate family, has a significant beneficial interest in any organization to which your physician refers you for services. Sunrise ASC is a physician owned surgical center and Dr. LeSueur is one of the physician owners. Please be advised that you may choose any other organization for the purpose of obtaining the services ordered or requested.
