Surgery patients who are having an urgent problem should call Dr. LeSueur at his emergency number which is listed on your post-op instructions. If you are having a true emergency, please call 911.

Release of Information

Due to privacy laws that have been enacted to keep your records private, we cannot release patient information without a signed request and a picture I.D proof of identity. If you wish to print the records release form from our website and mail it to us, it must be notarized. It is preferred that you have the form faxed to us at (928) 532-0078 from the doctor's office to whom you are requesting your information be released.​
